When immediate assessment may be needed

Call for urgent help after a severe head injury, loss of consciousness, a first or prolonged seizure, sudden weakness, new speech difficulty, severe sudden headache, repeated vomiting, confusion, or rapidly worsening limb weakness. Symptoms and severity can vary; a website cannot determine the diagnosis.

• Do not drive yourself if you are unconscious, confused or severely unwell.

Emergency assessment process

The clinical team may first assess breathing, circulation, consciousness, pupils, limb strength and vital signs. Imaging, laboratory tests, observation, admission, specialist evaluation or transfer may be recommended depending on the findings.

Head injury and loss of consciousness

Even an apparently minor injury may need urgent evaluation when there is loss of consciousness, worsening drowsiness, repeated vomiting, seizure, fluid or blood from the nose or ear, unequal pupils, weakness or use of blood-thinning medicine.

Stroke and brain haemorrhage warning signs

Use FAST: Face drooping, Arm weakness, Speech difficulty, Time to seek emergency help. A sudden severe headache, collapse, vomiting, seizure or new weakness may also require urgent assessment. Treatment depends on the cause and scan findings.

Seizure or sudden deterioration

Protect the person from nearby hazards, do not restrain them and do not place anything in the mouth. Seek emergency help for a first seizure, a seizure lasting around five minutes or longer, repeated seizures without recovery, injury, breathing difficulty or pregnancy.

Acute spine trauma

New weakness, numbness, loss of bladder or bowel control, severe neck or back pain after trauma, or difficulty walking can require emergency assessment and spinal precautions. Avoid unnecessary movement after significant trauma.

Emergency admission and hospital support

Emergency assessment may lead to observation or IPD admission. Operation theatre, ICU and ventilator requirements depend on clinical assessment and current capacity. Call before travelling when possible to confirm the appropriate arrival and admission pathway.
